  • TLC's Insurance Plans It is ultimately the responsibility of the patient to be aware of their benefits.  Although we do verify benefits for each visit, we do so as a courtesy and cannot guarantee coverage or payment by any insurance company.  We always suggest that patients check with their insurance company to be sure that our providers are in-network with their plan.  Patients are responsible for obtaining any necessary referrals.  If assistance is needed, we will gladly help.  All deductibles, co-insurance, and copayments will be due at the time of service.  This list may not be representative of all the insurance plans we accept and may change at any time.
