Disability and FMLA forms
TLC for Women charges an administrative fee to complete Insurance
Disability forms, FMLA (Family and Medical Leave Act) forms, and other
work-related forms. These forms require verification and signature by
a physician. The fee for this service is $15 per form. To expedite
this service please complete all sections that are required by you, the
employee, and sign and date all areas where required. Please bring
these forms and payment to one of our offices and we will complete the
remaining applicable sections. We will notify you when the form is
complete and we will be happy to fax or mail the form to the location
you specify. All forms take 7-10 business days to complete.
