How to obtain a copy of your medical records
To obtain a copy of your medical records, a HIPAA compliant medical
release form must be completed. (Please see the link at the bottom of the F&Q page.) Complete the
form and mail or fax it back to us. Be sure to include all of your
contact information so that we may reach you if necessary; and, be sure
to sign and date the form.
All requests take 7-10 business days to process. We will do our best
to complete requests that are needed sooner. We cannot fax any records
that are over 25 pages. These must either be mailed or picked up at
one of our offices.
Charges will apply. All charges must be paid before records are copied. For further information, please contact our office.
